Release or Informed Consent Forms (online or paper) are required for all ice facility use. SCC members have completed the forms as part of the membership.

These events include:

• Bonspiels
• Member / Guest Ice Bookings
• Practice Ice - Non Simcoe Curling Club Members
• etc. 

Please ensure that all participants in your event complete either the Release Form or Informed Consent Form:
